Fulminating Amoebic Colitis Associated with Multiple Perforation


Abstract
Amebiasis is a infectious disease produced by entamoeba histolytica. It occurs in all world but is more prevalent in both subtropical and tropical countries, and more frequent in males than in females.
The primary sites of lesions produced by E. historytica are located in the large intestine, mainly cecum, sigmoid and rectum, where it produces mucosal ulceration penetrating into submucosal and muscular layers. And secondary infections make the amebic ulcerations more extensive and result in perforation of the colon.
In case of perforation, surgical investigation is more effective than conservative management in which the mortality is almost 100%, but it also appears high mortality. A case described by authors was a 36 year old convict man jailed into Taejon prison, who was admitted to our hospital with chief complaints of abdominal pain, abdominal distension and vomiting for 3days duration. And he received emergency operation under the impression of the perforation of
hollow viscus according to clinical, physical and simple abdominal X-ray findings.
Operative findings showed the multiple perforations through-out entire colon and panperitonitis. Therefore total colectomy and ileostomy was performed and the results of biopsy reveals that mucosal ulceration is characterized by proteolytic type of necrosis with scanty exudative reaction and by deeply undermined, with penetration into deep submucosa. Many entamoeba historytica phagocytizing Rbc. are encountered. Pathological diagnosis was amebic colitis. The patient was expired on postoperative 10 the day.
